This is a statutory requirement of the Australian Skills Quality Authority (ASQA) and it is your responsibility to confirm that having read the Qualification Information you can undertake the course. Please read the application instructions carefully and complete the form before having it signed by your supervisor who is also required to confirm that the course is at a suitable standard for you.
